Remote Support, Face-to-Face Management
Our Clinical Pharmacist Services are primarily delivered remotely, saving practices the need for additional workspace while allowing clinicians to integrate with existing systems and workflows.
But our pharmacists aren’t simply left to work remotely in isolation. TMMT uses a face-to-face hub model to train, support and manage clinicians internally, with access to wider clinical and management support.
This means your practice can access additional clinical capacity without taking on the same training, supervision, performance-management and workforce responsibilities associated with employing an additional pharmacist directly.
Supporting Your Existing Pharmacy Team
TMMT doesn’t have to replace your existing pharmacy workforce.
Our clinical pharmacists can work alongside pharmacists and pharmacy technicians already employed by your practice or PCN, providing additional clinical support, capacity and help managing priorities such as medicines optimisation, QOF work and PCN targets.

109 Structured Medication Reviews Completed in Eight Days
When Elm Tree Medical Centre needed additional capacity for a defined Structured Medication Review project, TMMT mobilised a managed service around a cohort of 114 patients.
From request to completion, the project was delivered in eight days, with:
- 109 SMRs completed
- 95.6% completion rate
- 51 medicines stopped or deprescribed
- 57 dose optimisation interventions
- 63 medication monitoring interventions
- 104 eRD setups
- 109 repeat synchronisations
