The Medicines Management Team is a provider of remote clinical pharmacist and pharmacy technician services that support GP practices, Primary Care Networks (PCNs), and healthcare organisations. Our goal is to optimise medication management, reduce administrative pressure on GPs, and improve patient care across primary care settings.

The Medicines Management Team offers a wide range of medicines optimisation and clinical services including Long term condition reviews, COPD reviews, Asthma, HF, Diabetes, CKD, SMI, MHT medication reviews, polypharmacy reviews, prescription queries, medicines reconciliation, drug monitoring, and prescribing quality audits. Our also support practices with targets such as QOF, IIF, and structured medication reviews, etc.

Their services are designed for GP practices, Primary Care Networks (PCNs), and GP federations that need support with medicines management, prescribing workload, and clinical administration. By integrating pharmacists and pharmacy technicians into practices, they help free up GP time to focus on complex patient care.

TMMT pharmacists help manage medication-related workload by handling repeat and acute prescription requests, medication reviews, discharge reconciliation, and chronic disease reviews. Our can also support extended access services and help practices meet key NHS targets.

Yes. The Medicines Management Team pharmacists receive continuous clinical training and mentoring from senior pharmacists and GPs. They are supported through a remote pharmacy hub and ongoing education to ensure safe and effective patient care.

Yes. All The Medicines Management Team’s pharmacist and pharmacy technician services are fully covered by the Additional Roles Reimbursement Scheme (ARRS) budget. Practices and PCNs can use their ARRS allocation to access medicines optimisation support without additional costs.
