2 FTE PCN Pharmacist Support – 6-month SMR
TMMT PCN Pharmacist Support was provided to a PCN with a population size of 95,286 patients. TMMT provided the PCN with 2 x FTE PCN Pharmacists to complete a total of 5000+ structured medication reviews within a six-month period.

TMMT were able to deliver a total of 75 hours of pharmacist support to the PCN and ensured a total of 200 structured medication reviews were completed on a weekly basis for the whole duration of a 6-month period.
