Addressing ARRS Staff Challenges in General Practice: Insights and Solutions

Updated: Aug 27

ARRS Staff by TMMT

The Additional Roles Reimbursement Scheme (ARRS) was introduced to alleviate workload pressures in general practice by expanding capacity and addressing workforce shortages. However, a report from the Queen's Nursing Institute (QNI) suggests that ARRS may not be meeting its goals effectively. Key findings from the report, based on a survey of over 500 general practice nurses (GPNs), include:

  • Increased Workload: 37% of GPNs reported an increase in their workload due to ARRS roles.

  • Lack of Reduction in Workload: Only 24% experienced a workload decrease, while 67% saw no change.

  • Need for Support: Many ARRS staff require significant education and supervision, adding strain to the existing staff, often without corresponding pay.

  • Operational Challenges: ARRS roles often fill existing gaps rather than adding value, with many staff unable to work independently without additional training or input.

These challenges have led to "taskification," where care becomes disjointed and episodic, leaving GPNs to "rescue" the situation by completing care episodes or rectifying issues caused by inadequate ARRS staff training.

TMMT’s Managed Clinical Service Solution:

To combat these issues, The Medicines Management Team (TMMT) offers a comprehensive managed clinical service. Our approach includes:

  • Comprehensive Management: From initial induction to daily operations, including workflow supervision and auditing.

  • Continuous Training: TMMT's unique training hubs, led by experienced GPs and senior pharmacists, provide ongoing upskilling and support, reducing the burden on PCN and GP practices.

  • Customised Support: We tailor our services to meet the specific needs of each practice within a PCN, ensuring effective integration and operation.

  • Regular Updates: Practices receive updates on achievements, targets met, and comprehensive audit and supervision logs.

By addressing these critical areas, TMMT helps streamline ARRS staff integration and enhances the overall effectiveness of primary care services.
